The inspector’s account

On 10/16/2025, Licensing Program Analyst (LPA) Pamela Bunker conducted an unannounced Case Management Visit to the above facility. LPA Bunker met with staff members, KoSarachi Ezeibe. LPA Bunker spoke with Licensee, Brenda Chandler, and Administrator Kim Post via telephone and explained the purpose of the visit. LPA was granted entry to the facility.

LPA Bunker and staff member KoSarachi Ezeibeand toured the facility's buildings and grounds to observe and identify any signs of neglect, abuse, or other immediate health and safety threats. No signs of neglect or abuse were observed during today's visit.

LPA Bunker requested and reviewed the facility's Emergency Disaster Plan, the Sleep Record, and the Fire Watch Record. LPA Bunker observed the facility to be clean, safe, sanitary, and in good repair. LPA Bunker observed that the facility was fully staffed at the time of the visit.

Ms. Chandler stated that Building and Safety had returned a request for corrections, specifically asking for the number of non-ambulatory beds and specifications regarding the fire door. She reported that the requested information was submitted on Tuesday, October 14, 2025, and that she plans to follow up in person on Monday, October 20, 2025.

No deficiencies were cited during the visit.
