Facility Evaluation Report
On 4/10/2025, At 1:10 PM, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct annual comprehensive inspection. Upon arrival LPA introduced herself to Shirley Jones and explained the purpose of the visit and was allowed entrance into the facility.
The facility is a single-story-family home located in a residential neighborhood. LPA Allen verified that the facility has an approved mitigation and infection control plan. The facility is licensed for the capacity of ten (10) adults ages 18-59, for mentally disabled clients. The current census is ten (10).
The facility consist of Living room, office area, 2 kitchens, dining area, 5 bedrooms, 2 bathrooms, freezer/storage room, laundry area in the garage, shaded area, indoor/outdoor activity areas, and a detached garage. The front and back yard landscape is in good condition at the time of visit.
At 1:25 PM, LPA Allen and Shirley Jones toured the physical plant. There were no bodies of water or obstructions on the premises. The beds and bedding supplies were in good condition, adequate lighting is provided, and there is storage space for client’s personal belongings. LPA Also observed additional bed linens, comforters, and bath towels were adequately stocked at the time of visit. The bathrooms were clean and operational. The water temperature ranged from 105 -117.5 F° and the facility temperature ranged from 72-75 degrees F.
At 2:15 PM, LPA Allen observed that facilities kitchen to have a 5-day supply of perishables and a 7-day supply of non-perishables food items for the number of clients in care which were stored and maintained properly. There was also a menus available for review.
