The inspector’s account

On 07/08/2026, at 9:45 am, Licensing Program Analyst (LPA) Antonine Richard conducted an unannounced annual required inspection visit to Brooks Place II. LPA met with Charles Flemings, Licensee, and explained the purpose of today’s visit. The facility is licensed to serve (4) Developmentally Disabled clients ages 18 to 59. Currently, the home has (2) clients. The clients are Westside Regional Center clients. None of the clients have Restricted Health Care Conditions and none are utilizing postural supports or protective devices. The facilities' annual fee is past due.

The facility is a single-story residential home located in a residential neighborhood. The home consists of the following: 4 bedrooms, 2 bathrooms, family room, dining room, kitchen, living room, shaded patio area, laundry room, and garage.

LPA conducted a records review of (2) client records, (2) staff records, (0) clients Personal & Incidental Records and reviewed the facility disaster plan. The facility disaster plan was current and in compliance with Title 22 at the time of visit.
