Facility Evaluation Report
On 05/29/26 Licensing Program Analyst (LPA) Mario Leon conducted an unannounced required 1-year annual visit using the CARE Inspection Tool. Upon arrival at the facility, California Department of Social Services (CDSS) met with Beverly Creel - Licensee and explained the purpose of today's Annual Inspection. California Department of Social Services (CDSS) verified that the facility has an approved mitigation plan report and infection control plan. There are currently three (3), Westside Regional Center Adult Residential Care Facility (ARF) consumers in placement.
The facility is a single-family home located in a residential neighborhood. Administrator Beverly Creel and CDSS toured the facility which consisted of the following: Living room, dining room, kitchen, breakfast area, staff room/den, 2 bedrooms, 1.5 bathrooms, laundry room, shaded patio area, attached garage, and indoor/outdoor activity areas.
Documents are posted as mandated on the walls and the dining room and the fireplace mantel. Bedrooms contain the furniture mandated, and bathrooms are clean and operational. Personal accommodations were observed for safety, privacy, comfort, and non-skid surface mats. The kitchen was observed for its ability to prepare and serve food. The food service was reviewed for appropriate quantity and proper storage; there was an ample supply of perishable and nonperishable food. The resident’s medications were reviewed for proper storage, documentation, and system implementation. Medications are in a locked cabinet in the kitchen, and records are current. Common areas were observed for the ability to safely serve the needs of the clients, including cleanliness, and clearness of any potential hazards to the residents. The first aid kit is fully stocked with a manual, one (1) smoke detector was in disrepair and licensee has agreed to repair. Carbon monoxide detector was in compliance, the hot water temperature was measured at 115.7°F (degrees Fahrenheit) within the normal limits (105-120F degrees).
