Facility Evaluation Report

At 10:28 AM Licensing Program Analyst (LPA) Bernadette Allen conducted an announced visit to conduct an annual inspection. LPA Allen was greeted by Osayande(Greg) Aghazebamwan- Licensee who was informed of the purpose of the visit and was allowed entry into the facility.

The facility is licensed as a Adult Residential for ages 18-59 for a capacity of four (4) ambulatory residents only. There are currently two (2) residents who were at day program during the visit.

The facility is a single-family home located in a residential neighborhood with four (4) bedrooms, two (2) full bathrooms, a living room, dining area, kitchen, and laundry area located in the kitchen. The facility has a patio/shaded area and, an indoor/outdoor activity area located in the backyard. There are two (2) detached car garages located in the back of the property.

At 10:45 AM, LPA Allen and Osayande (Greg) Aghazebamwa toured the physical plant. There were no bodies of water or obstructions on the premises. The beds and bedding supplies were in good condition, adequate lighting is provided, and there is storage space for residents personal belongings. LPA Allen also observed additional bed linens, comforters,and bath towels.

The bathrooms were clean and operational with the water temperature ranging from 105 -117.5 F° and the facility temperature was 76 degrees F° .
