Allegation: Staff do not provide adequate food service
On October 31, 2025, between 9:45 a.m. and 11:30 a.m., the Department conducted a joint interview with Staff #1 and Staff #2 (S1–S2). Both staff members (2 out of 2) stated that adequate food service is provided. Clients receive three meals per day plus snacks: breakfast at 7:30 a.m., lunch at 12:00 p.m., dinner at 4:30 p.m., and snacks at 7:30 p.m. Clients are allowed to have second servings. During the visit, an ample supply of nonperishable food items sufficient for at least one week was observed, along with fresh perishable foods adequate for a minimum of two days. S1–S2 reported that none of the clients had experienced food-related illnesses and confirmed that spoiled food is not served. They stated that there were no bugs in the beans. Both staff members (2 out of 2) states that they serve healthy meals, with most food prepared from scratch. The Department reviewed the facility’s food menu for the period of October 1, 2025, through October 31, 2025. During today's visit, LPA observed clients eating lunch and confirmed that meals were being served in accordance with the posted menu. S1–S2 also reported that clients have not complained about the food. Both S1 and S2 denied the allegation.
On October 31, 2025, between 11:30 a.m. and 1:00 p.m., the Department interviewed Clients #1 through #5 (C1–C5). When asked whether staff provided adequate food service, all five clients (5 out of 5) stated that staff do provide sufficient food and that they receive plenty to eat. All five clients confirmed they are served three meals a day—breakfast, lunch, and dinner—along with snacks. They reported that breakfast is served at 7:30 a.m., lunch at 12:00 p.m., dinner at 4:30 p.m., and snacks at 7:30 p.m. Clients also noted they are allowed second servings if desired. Each of the five (5 out of 5) clients stated they had not experienced any food-related illness, stated that the food was not spoiled, confirmed there were no bugs in the beans, and that they were satisfied with the meals provided. All five clients (5 out of 5) denied the allegation and stated that staff provide adequate food service.
Allegation: Staff Are Mistreating a Client
On October 31, 2025, between 9:45 a.m. and 11:30 a.m., the Department conducted a joint interview with Staff #1 and Staff #2 (S1–S2). Both staff members (2 out of 2) stated that staff do not mistreat any of the clients. They reported that clients are provided with a safe, healthful, and comfortable environment, and that staff offer the necessary care and supervision to meet each client's needs.
