Facility Evaluation Report
Licensing Program Analyst (LPA) Pamela Bunker conducted an unannounced, annual required one-year visit, with a primary focus on infection control measures and the implementation of the new CARE Inspection Tool.
Upon arrival at the facility, LPA Bunker completed a risk assessment. Based on the evaluation, the facility was found to be clear of COVID-19 infection. LPA was properly screened for COVID-19 symptoms, and her temperature was checked. LPA Bunker met with Administrator Mehul Patel and explained the purpose of the annual inspection. LPA verified that the facility has an approved Mitigation Plan Report and Infection Control Report. There are currently fifteen (15) residents in placement.
Administrator Mehul Patel and LPA Bunker toured the facility, which is a two-story converted hotel located in a business area. The building consists of 18 bedrooms, 18 bathrooms, a kitchen, dining room, laundry room, storage room, seven detached carports, a shaded area, and indoor/outdoor activity areas. During the visit, Mr. Patel and LPA Bunker observed residents’ bedrooms and bathrooms.
Due to time constraints, LPA was unable to complete the full inspection and will return at a later date to finalize the visit.
No deficiencies were cited during this visit. An exit interview was conducted.
