On 08/29/2025 at 2:34 p.m., Licensing Program Analyst (LPA) Pamela Bunker conducted a case Management visit to amend LIC9099-C dated 05/19/2025, for the Complaint Report dated 05/14/2025, Control # 11-AS-20250514123624, and to provide clarification of the findings, which do not change them. LPA Bunker met with Administrator Mehul Patel and explained the purpose of today's visit. LPA was granted entry to the facility.
