Facility Evaluation Report

On March 3, 2025, Licensing Program Analyst (LPA) Alfonso Iniguez conducted an unannounced Case Management visit. The LPA met with Franca Okere, the administrator, and explained the purpose of the visit.

On May 7, 2024, the Department conducted an initial visit regarding complaint number 11-AS-20240508133113. This visit was rendered needs further. On March 3, 2025, LPA Iniguez conducted an unannounced subsequent complaint visit to deliver findings regarding complaint number 11-AS-20240508133113; since the findings were Substantiated, LPA Iniguez rendered citations. On the other hand, since the incident happened almost a year ago, the administrator already conducted plan of correction on May 15, 2024. LPA Iniguez cleared the citation rendered on March 3, 2025.

An exit interview was conducted, and Franca Okere, the administrator, received a copy of this Case Management report.
