Facility Evaluation Report

Licensing Program Analyst (LPA) Pamela Bunker conducted an unannounced Case Management. LPA Bunker met with staff member Melissa Cortez and explained the purpose of today's visit. LPA was granted entry into the facility. We toured the facility buildings and grounds to observe and identify any signs of neglect, abuse, or other immediate health and safety threats. We did not observe any signs of neglect or abuse during today's visit. During the visit, LPA Bunker spoke to Licensee Delores Davis via telephone. The Licensee, Ms. Davis, stated that she was not available to meet today and requested that LPA Bunker return at a later date to discuss the complaint allegation.
