Facility Evaluation Report

Licensing Program Analyst (LPA) Pamela Bunker conducted an unannounced case management visit. LPA Bunker met with staff member Mollie Davis and explained the purpose of today's visit. LPA was granted entry into the facility.

The facility’s buildings and grounds were toured to observe and identify any signs of neglect, abuse, or other immediate health and safety threats. No signs of neglect or abuse were observed during today's visit.

During the visit, LPA Bunker spoke with Licensee Delores Davis by telephone. Ms. Davis stated that she fulfilled the payment plans with both families in accordance with the agreed-upon terms. She submitted proof of the final payment, along with an Incident Report, to the California Department of Social Services.
