Facility Evaluation Report
Licensing Program Analyst (LPA) Jeremiah Randle arrived to conduct an unannounced Annual Required inspection to the above Adult Day Program (ADP). Upon arriving at the facility LPA met with Eduardo Martinez Program Director and informed him of the purpose of the visit. The facility is licensed to serve forty-five (45) Developmentally Disabled clients, ages 18 - 59 years. (45) Ambulatory. Staff to Client ratio is one (1) staff to three (3) clients.
LPA Jeremiah Randle along with Eduardo Martinez Program Director inspected physical plant inside and out. The day program room setting is equipped with tables, chairs, exercise equipment area, and a small kitchenette area in the back of the facility. LPA observed sufficient furniture and lighting throughout the facility. There are no bodies of water present in the facility. Care and Supervision necessary to meet the client's needs and all services specified in the admission agreements are being met.
LPA observed toxins are kept locked and inaccessible to clients. Clients bring lunch to day program, program provides snacks. The hot water temperature was measured and is within Title 22 Regulations (118 F). The restrooms (2) were clean and maintained in a safe, sanitary, operating condition and are sufficient for staff and clients and meet Title 22 regulations. Fully charged fire extinguishers (2) and smoke detectors are current and in compliance with fire safety. Carbon monoxide detector is operable and meets statutory requirements. A comfortable temperature is maintained within facility.
Facility does not handle and store medication, however there is a 2 bed Nurse's Station partitioned for resident safety. LPA reviewed client and staff files, including criminal record clearances. First aid kit with manual was checked and is complete.
The day program facility was free of odors. The program administration develops, maintains and implements written plans for orientation, on-the-job training.
According to the California Code of Regulations (Title 22, Division 6, Chapter 8), LPA did observe a deficiencies, therefore no citations were issued at this time.
