Facility Evaluation Report
Licensing Program Analyst (LPA) Glenn Trueman made an unannounced visit to the facility and was greeted by Denise Montes Office Manager and explained the reason for the visit. Shortly thereafter Administrator Gislene Kouesseu arrived.
The purpose of the visit is to conduct an Annual Inspection visit.
The visit included the following domains:
Infection Control, Physical Plant/ Environmental Safety, Operational Requirements, Personnel Records- Training, Client records- incident Reports, Client rights/ information, Food Service, Health Related Services, Incidental Medical Services and Disaster Preparedness.
LPA toured the facility along with Denise Montes Office Manager and the following was observed:
Day Program has a main lobby and 5 offices.
There are 6 tables with 12 computers for Job skills.
There is a conference room which is also used for counseling and as an activity room.
Kitchen area has a cabinet with snacks. At this time facility is not serving food or administering medication.
In the kitchen area are 2 refrigerators and a cabinet for locked chemicals.
Program has all the necessary postings throughout the program.
In main Job Skills room there are cabinets for activity materials and a locked up cabinet for PPE.
Facility contains a carbon monoxide detector.
Program has an Emergency and Disaster Plan.
An emergency exiting plan and emergency phone numbers posted in an appropriate place.
An operating telephone on the premises and available to clients.
First aid supplies, which include sterile first aid dressings, bandages, adhesive tapes, scissors, tweezers, thermometer, antiseptic solution, and a current first aid manual is maintained.
4 staff files were reviewed and 3 client files were reviewed.
No deficiencies. Exit interview conducted and copy provided.
