Facility Evaluation Report

Licensing Program Analyst (LPA) Glenn Trueman conducted an unannounced Case Management visit. The purpose of the visit was to follow-up on the facility closure request. The department received written notification of facility closure as of 5/31/26.The purpose of the visit was explained to House Manager Robert Otero.

Facility did not have the original license, but submitted an older license that listed the capacity as 12.

Interview was conducted with House Manager Robert Otero who stated there are still 2 clients here Client C1 and Client C2 and stated that they are not provided care and supervision.

Interview was conducted with Person (P1) telephonically.

Review of Client C1 and Client C2's file was done and various documents were submitted.

At this time it has been determined that further investigation is needed.

An exit interview was conducted and a copy of the report was issued to House Manager Robert Otero.
