Facility Evaluation Report
On 05/01/26, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced annual visit using the CARE Inspection Tool. LPA met with Executive Director Judith Uy Villaruz and explained the purpose of today’s visit. The facility is licensed to serve one hundred forty-two (142) non-ambulatory residents, of which six may be bedridden. The facility has a hospice waiver for ten residents.
During today’s visit, LPA toured the inside and outside of the facility.
Due to insufficient time, an annual continuation is required.
An exit interview was conducted and a copy of this report was reviewed and discussed with the Executive Director Judith Uy Villaruz.
