Facility Evaluation Report

On 04/16/2026, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with Program Manager Mark Rodriguez. LPA explained the purpose of the visit and was accompanied by Program Manager inside the facility during this inspection. The Day Program is licensed to serve 52 developmentally disabled clients, 8 non-ambulatory and 44 ambulatory. Clients are linked to both the West Side Regional Center and the Harbor Regional Center. Facility fees are current.

The Day Program is a single-story structure located in a community center within a residential neighborhood. Rooms #15 and #17 are utilized by the Day Program. Room #15 is an activity room with a kitchen and Room #17 is an activity room that includes a computer room. There are two restrooms which are shared with the Community Center.

The Day Program walls and floors were in good condition, adequate lighting, fire extinguishers were properly charged. Plenty of storage space for clients’ belongings, sharps, and toxins were properly locked. Water temperature measured at 111.5 degree Fahrenheit. The men's and women's restrooms were clean and within Title 22 regulations. Bottled water is available for clients. The kitchen was clean and a refrigerator was available for client use, the day program does not provide lunch. Continue to LIC809-C.
