The inspector’s account

On 04/23/25 Licensing Program Analyst (LPA) Villegas conducted an unannounced required annual visit using the CARE Inspection Tool. LPA met with Day program Program Manager Mark Rodriguez as the purpose of the visit was explained. The Day Program is licensed to serve 52 developmentally disabled clients, 8 non-ambulatory and 44 ambulatory. Current census is 14 clients, clients are linked to both the West side regional center and the Harbor Regional center. The staff to client ratio is 1 staff to 4 clients. Facility fees are current.

The Day Program is a single story structure located in a community center within a residential neighborhood. Rooms #15 and #17 are utilized by the Day Program. Room #15 is Activity room with a kitchen, Room #17 is Activity room that includes a Computer room. There are 2 restrooms which are shared with the Community Center. Day Program walls and floors were in good condition, adequate lighting, fire extinguishers were properly charged. Plenty of storage space for clients belongings, sharps, and toxins were properly locked. Water temperature measured at 114.8F. The men's and women's restrooms were clean and within Title 22 regulations. A water dispenser was available for clients use. The kitchen was clean and a refrigerator was available for client use, the day program does not provide lunch. The first aid kits were available; the facility does not administer medications. Walkways throughout the day program were clear of hazards and all exits were clear of debris, there are no bodies of water on the premises. LPA observed the day program to be sanitary and appropriately supplied at the time of visit. A landline and internet service was observed. The last fire drill conducted was on 02/11/25

LPA conducted a review of 7 client records, and 3 staff records, no discrepancies observed.
