The inspector’s account

On 08/07/2025, Licensing Program Analyst (LPA), Wendy Gibbs, conducted an unannounced Annual Visit to the facility listed above. LPA met with Staff, Estela Briseno, and the purpose of today’s visit was explained. We were later joined by Administrator, Robert Woodyear. LPA was granted entry into the facility.

Files LPA reviewed the file for three (3) clients and observed they contained the required documents. During file review, LPA observed licensing fees are current. The Administrator’s Administrator Certificate is valid till 06/08/2026.

Safety LPA observed the facility has an Emergency Disaster Plan posted on a board on the wall. LPA observed the exits are marked with an EXIT sign. LPA observed the facility has a working landline telephone. LPA inspected the First Aid Kit and observed it had the required items.

Medications LPA observed centrally stored medication secured in a locked cabinet in the hallway. All medications were observed in their original packaging. LPA reviewed the Centrally Stored Medications and Medication Administration Records (MAR) for three (3) Clients. LPA observed three (3) out of three (3) Client’s medications were consistent with properly documented records.

No deficiencies were observed or cited during today’s visit. LPA will return at a later date to complete the facility evaluation.

An exit interview was conducted with Administrator, Robert Woodyear, and a copy of this report was provided.
