Facility Evaluation Report
On 07/10/2024 around 10:00 AM Licensing Program Analyst (LPA), Leandro conducted an announced visit to the facility for purpose of a pre-licensing evaluation. LPA met with Licensee, Pamela McClendon.
On 11/20/2023 an application was submitted to CCLD, for Initial license for an Adult Residential Facility to serve adults age range 18 through 59. The requested capacity is for 4 ambulatory clients.
The facility is a one-story house located in a residential street. The facility has 4 client bedrooms, 2 bathrooms, 1 kitchen/dinning room/living room area, 1 day room (with a large mounted tv, games for clients, a laundry area, and an electric fireplace), 1 detached garage, 1 office, 1 backyard area with shaded seating. The front entrance has a wheelchair ramp.
LPA Leandro conducted a review of the Physical Plant, Bedrooms, Bathrooms, Supplies, Food Service, Medications, Records, Administration, Activities, Pe-Licensing Checklist and Component III Orientation.
There is a locked centralized storage area for client medications.
Facility is clean, sanitary, and in good repair. Indoor and outdoor passageways, stairways, inclines, ramps, open porches, and other areas of potential hazard are free of obstructions. All window screens are clean and in good repair. Facility temperature is between 68 degrees and 85 degrees. Fireplaces and open-faced heaters are inaccessible to clients. Stairways, inclines, ramps, open porches, and areas of potential hazard are well-lit and equipped with sturdy hand railings. For facilities of 16 or more capacity there is a private office for the administrator, a reception area, and bathroom for visitors. Fire Alarms and Smoke alarms operate properly. Carbon monoxide detectors operate properly.
