Facility Evaluation Report
On 04/16/2026 at 08:30am, the California Department of Social Services (CDSS) Community Care Licensing Division (CCLD) conducted an unannounced required – 1 year inspection to the above-named facility and met with Detric Johnson (DSP/House Manager). The Department explained the purpose of the visit. This facility is licensed to serve 3 non-ambulatory adults ages 18 to 59 years old, of which 3 maybe on hospice. Two (2) of the clients currently have an Age Waiver. A total of 3 clients are currently residing in this facility.
The facility has a current administrator certificate for Janice Lofton (#7034359735) valid 04/18/2025 - 04/17/2027. The facility fees a balance of $681.00 due on 03/22/2026. LPA provided the facility with the PIN # 911170 with the option to make online payment. The facility made annual fee payment (confirmation #902735) on 04/16/2026.
The facility is a one-story house located in a residential street which consists of 3 client bedrooms, 2 full bathrooms, 1 living room area, 1 kitchen area, 1 dining room area, 1 tv room, 1 detached garage with an additional refrigerator and emergency disaster food and supplies with an attached office, and 1 backyard with an outdoor patio area with shaded area.
Between the hours of 8:40am - 10:00am, LPA conducted a records review of (3) client records, (5) staff records, (3) Client Medication Administration Records, (3) clients Personal & Incidental Records, and the facility disaster plan. All client & staff records were complete. The facility last disaster drill was conducted on 03/12/2026 and in compliance with Title 22 at the time of visit. LPA did not observed any discrepancies at the time of visit.
