Facility Evaluation Report
On 07/03/2025, Licensing Program Analyst (LPA) Socorro Leandro conducted an unannounced Case Management visit. LPA met with Administrator, Janice Lofton and explained the purpose of the visit. LPA was allowed entrance to the facility.
The facility has an approved fire clearance for 3 non-ambulatory clients and approval for 2 client to wear a helmet.
There are ramps in the front entrance and back exit of the facility.
There are currently 3 clients in the facility.
3 clients physicians reports were reviewed and a facility tour was conducted.
An exit interview was conducted a copy of this report was left with the Administrator, Janice Lofton.
