Facility Evaluation Report
On 05/08/2025 around 10:00 AM, the California Department of Social Services (CDSS) – Community Care Licensing Division (CCLD) Licensing Program Analyst (LPA) Socorro Leandro conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with Administrator, Janice Lofton. LPA explained the purpose of the visit to Direct Support Staff, Stacy Luise Tsuboi and was allowed entry to the facility.
This facility is licensed to serve 3 non-ambulatory adults ages 18 to 59 years old, of which 3 maybe on hospice. Two clients currently have an Age Waiver.
A total of 3 clients are currently residing in this facility.
The Annual Licensing Fees are current. On 05/08/2025, the facility paid their Annual Licensing Fees.
