Facility Evaluation Report

On 04/08/2026, the California Department of Social Services (CDSS) – Community Care Licensing Division (CCLD) Licensing Program Analyst (LPA), Socorro Leandro conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with the Director, Bridgett Franco and was granted entrance into the facility. LPA explained the purpose of the visit and was accompanied by the Director inside and outside the facility during this inspection.

This facility is licensed to serve a total of 45 adults ages 18 to 59 years old, of which 2 may be non-ambulatory.

Facility Layout: The facility is a one-story industrial building located on a main street.

The building consists of: 1 entrance lobby; 2 meeting rooms; 6 offices; 2 men’s bathrooms; 2 woman’s bathrooms; 1 isolation/sick room; 2 music rooms; 1 art room; 1 electrical room; 2 kitchen rooms; 1 pantry room; 1 break room; 2 storage rooms; 1 large activity room; 1 lobby/meeting room area; 1 outside fenced area with large trash bin; 1 outside fenced area with recycling bins; 1 outside garden with shaded seating; and 1 outside parking area.
