Facility Evaluation Report
On November 14, 2022, Licensing Program Analyst (LPA) Wendy Gibbs conducted an unannounced Annual inspection visit at the Mychal's Learning Place Adult Day Program. LPA Gibbs met with (Administrative Program Director) Olivia Milton and Prescila Tran and explained the purpose of the visit. Currently, there are a total of 37 consumers in the Adult Day program. There is a total of 27 consumers physically at facility and the remainder are virtual. The Adult day program hours are Monday-Friday, from 8:30am-2:00pm.
LPA toured the facility with Olivia and Prescila and observed the following: Entrance door leads to waiting area and receptionist/ information desk, 5 administrative offices, Back Room for clients to log in with review of daily activities, 2 conference rooms (1 conference room is also a resource center), multi-purpose activity area/ room, commercial kitchen, laundry room, art room, quiet room (mock bedroom), music room, women and men restroom. There is a large room "Big Purple Room", used for fitness/exercise and other activities (mainly used so consumers can spread apart).
The kitchen was fully stocked. The hot water temperature measured 119.5 degrees Fahrenheit. All appliances were in good working condition. All knives are locked in a drawer. All toxins and cleaning supply are locked in cabinet.
Each restroom has two stalls inside. The toilets and sinks are operable and are handicap accessible. The hot water temperature measured between 113.2-118.3 degrees Fahrenheit in the women’s restroom and between 114.9-119.5 degrees Fahrenheit in the men’s restroom.
