On 07/10/2025, Licensing Program Analyst (LPA) Socorro Leandro conducted an unannounced Case Management visit. LPA met with the Administrator, Janiece Spears and explained the purpose of the visit. LPA was allowed entrance to the facility.
The facility has an approved fire clearance for 3 non-ambulatory clients.
There are ramps in the front entrance and back exit of the facility.
There are currently 2 clients in the facility.
2 clients physicians reports were reviewed, and a facility tour was conducted.
An exit interview was conducted a copy of this report was left with Administrator, Janiece Spears.
