Facility Evaluation Report

On 06/11/2024 around 9:40 AM, the California Department of Social Services (CDSS) – Community Care Licensing Division (CCLD) Licensing Program Analyst (LPA) Socorro Leandro conducted an unannounced continuation Required – 1 Year Inspection to the above-named facility and met with Administrator/Licensee, Allen Hawkins. CCLD staff explained the purpose of the visit and was accompanied by 3 staff inside and outside the facility during this inspection.

This facility is licensed to serve 4 ambulatory adults ages 18 to 59 years old.

Facility Layout: The facility is a one-story house located in a residential street. The home consists of 1 office room, 1 living room, 1 kitchen, 1 den/dining room, 2 client bedrooms, 1 staff bedroom, 1 detached laundry room, 1 outside patio area with shaded seating, and 1 accessory dwelling unit (ADU). The ADU is currently under construction.
