Facility Evaluation Report

On 07/10/2025, Licensing Program Analyst (LPA) Socorro Leandro conducted an unannounced Case Management visit. LPA met with LVN, Soala Briggs and explained the purpose of the visit. LPA was allowed entrance to the facility.

The facility has an approved fire clearance for 4 non-ambulatory clients and approval for postural support with safety rails, padded side rails for bed, padded lap tray for wheelchair and seatbelt for wheelchair. Approval for adult over age 65.

There are ramps in the front entrance and back exit of the facility.

There are currently 4 clients in the facility.

4 clients physicians reports were reviewed and a facility tour was conducted.

An exit interview was conducted a copy of this report was left with LVN, Soala Briggs.
