Facility Evaluation Report

On November 3, 2025, at around 11:40 AM, an office meeting was held to discuss additional civil penalties for a complaint that concluded on September 13, 2022 alleging "Facility did not seek timely medical attention for resident" and "Facility did not ensure that resident's wounds were kept clean" at ATAP Residential Home's former location with license number 198603159. Present at the meeting were Ulysses Coronel, Licensing Program Manager (LPM); Socorro Leandro, Licensing Program Analyst (LPA); Cori Waters, Administrator / Licensee; Jermaine Fletcher, Direct Support Provider (DSP).
