Facility Evaluation Report
On 10/22/2025, At 8:30 AM, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct annual comprehensive inspection. Upon arrival LPA introduced herself to Oscar Pascu- Support Staff and explained the purpose of the visit and LPA was allowed entrance into the facility. Morkodh Chhuon- Administrator arrived at 9:00AM and he was informed of the purpose of the visit.
The facility is a single-story-family home located in a residential neighborhood. LPA Allen verified that the facility has an approved mitigation and infection control plan. The facility is licensed for the capacity of four (4) adults aged 18-59, Ambulatory.
The facility consists of the following: four (4) client rooms, one (1) staff room, three (3) bathrooms, one (1) living area, one (1) family room, a dining area and kitchen. There is an outside shaded patio area in the backyard with a table and ample seating. The laundry area located in hallway space next to the dining room. There is a two (2) car garage detached and was empty with no cars parked in garage at the time of the visit and also used for storage.
At 9:05 AM, LPA reviewed two (2) staff files for First Aid/CPR certification, criminal record clearance, trainings, and health screenings, which appeared to be current during the inspection.
LPA reviewed two (2) clients files for admission agreements, updated physician reports, needs and services plans which appeared to be up to date during the inspection.
