Facility Evaluation Report
Licensing Program Analysts (LPAs) Cynthia Chan and Jewel Baptiste conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPAs arrived unannounced and met with Staff, Lindo Bhembe. The purpose of the visit was explained. The license is approved for five (5) ambulatory adults, ages 18 to 59. The facility is vendorized by the San Gabriel/Pomona Regional Center.
The facility has 4 bedrooms, 2 bathrooms, living room, dining area, kitchen, family room, laundry room, and an attached garage. The outdoor has a shaded area with table and chairs. Client bedrooms have the required furniture, sufficient lighting, and closet space. There are no items obstructing the walkways. There is no swimming pool on the premises. Facility has operable smoke and carbon monoxide combo detectors. LPA observed 2 day of perishable and at least a week of non-perishable food items. The hot water temperature is measured within the required range of 105-120 degrees F. Knives and sharps are locked in the kitchen cabinet. Cleaning supplies are stored locked in the garage. Staff stated they are continuing to follow the guidelines and cleaning everyday. LPA reviewed (3) personnel files. The administrator's certificate expires on 11/1/27 and the HIV & TB training were completed on 11/17/25. Staff have current CPR & First Aid certificates and receiving annual training. LPA reviewed (5) client files. Client have the required documents such as the admission agreement, consent forms, physician's report with TB results, client rights, and property valuable forms. P&I ledgers were also reviewed. Clients do not have any prohibited or restricted health conditions. Medications are centrally stored and being given as prescribed. The emergency disaster plan is updated and drills are being conducted.
No deficiencies issued today. An exit interview was held with D. Heller. A copy of this report was provided.
