Facility Evaluation Report

Licensing Program Analyst (LPA) Cynthia Chan conducted a case management visit regarding the construction project to the facility. LPA met with Project Manager, Matthew Riddell, and explained the purpose of the visit.

On 9/5/25, LPA received notice that the facility had planned a construction project that will begin on 9/9/25.

A virtual meeting was held on 9/8/25 to discuss the documents needed in order to proceed with the project of construction to bedroom #1. During the visit today, LPA did not observe any health and safety concerns and no construction has begun. Mr. Riddell stated that the project will not start until the plan is approved by the licensing department. Regional Center is aware of this project and supports the construction.

In addition, LPA observed that the current administrator (Aaron Yarbrough) does not have a valid administrator's certificate. It is confirmed that the renewal documents were submitted on 5/5/25 and is pending review by the Administrator Certification Bureau. LPA has not received the change of administrator documents for Aaron Yarbrough.

The following items were requested during the visit today:

* Facility plan for the construction project

* Documents for Aaron Yarbrough - Board Resolution, Personnel Record, health screening, TB test result, First Aid & CPR certificates

An exit interview was held. A copy of this report was given to Mr. Riddell.
