Facility Evaluation Report
On 12/15/25, at 9:15am , Licensing Program Analyst (LPA) Gina Saucedo arrived at the facility to conduct an unannounced, case management due to changes to facility sketch. The final approval was conducted by the Department of Building and Safety on 08/25/25. The facility now has two (2) staff bedrooms in the back, the dining hall was moved towards the back of the facility, there Is another bathroom and another client room towards the back of the facility, there is an office at the front entrance and another bathroom added next to room number four (4). There is a total of four (4) bedrooms for clients and three (3) bathrooms with two (2) staff bedrooms with no garage but there is a carport. The original sketch had a garage with one (1) staff room and no office. The original LIC 200 was for six (6) clients. On 08/30/23 there was also a capacity change to four (4) clients, and the LIC 200 was sent to CCLD-Community Care Licensing Department with a new facility sketch change. The Fire Inspection’s final approval was conducted on 08/29/25 with an updated LIC 850 and new LIC 200 was sent to the Community Care Licensing Department on 08/28/25 and the Pre-Inspection/Consultation Request of 1146.00 was paid on 01/22/25. All the documents have been paid for and updated for the above facility.
A copy of all the documents has been provided to the Licensee/Administrator, an exit interview was conducted, no citation(s) were issued, and a copy of this report was given to the licensee/Administrator.
