Facility Evaluation Report
Licensing Program Analyst (LPA) Michael Cava attempted to conduct a Required Annual visit at the facility. At arrival, LPA waited at the front door for approximately ten minutes but no answer. LPA called telephone number on file, and was able to get a hold of the administrator, Eduard Hakobyan. He stated his application for the Assisted Living Waiver (ALW) was just approved, but there are no residents currently in the facility. He adds he would like to continue to carry his license with the state, but facility is not operating at this time.
LPA observed the home from the outside. Property appears to be vacant, and not operating. No cars parked in driveway, and trash was empty.
LPA will make another visit to facility on a later date to complete annual. This report will be left at the front of the home, and will be mailed. Note that on previous annual visits, the facility also had zero residents as the licensee was awaiting their approval for the ALW program.
