Facility Evaluation Report
Licensing Program Analysts (LPAs) Gina Sauceda and Michael Cava conducted an Annual Required visit and inspection of the facility. LPAs met with staff, Elvis Austins and explained the reason for the visit. The administrator, Jose Oyinlove, joined the visit shortly.
At 09:52am, with the assistance of staff, LPAs took a tour of the physical plant. Required postings were observed in the entry area. The smoke alarms are hardwired and interconnected. There is a carbon monoxide detector located in the hallway, between resident rooms, that functions properly. The fire extinguisher is located in the kitchen. It is fully charged, but charge date is due soon. LPAs advised the administrator to follow up.
Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility; properly stored. Knives and sharps are stored in a locked closet away from the kitchen. Cleaning supplies and toxins are locked underneath the kitchen sink.
Bedrooms: There were a total five (5) bedrooms. Four (4) are designated for residents' use. One (1) is for staff. The bedrooms designated for resident use are private. All four rooms, in use by residents were properly furnished with appropriate beddings and linens with sufficient lighting.
Bathrooms: There are three (3) bathrooms. Two are designated for residents' use, and one is reserved for staff.. Both bathrooms, for the use of the residents, were properly supplied and had functional fixtures. Hot water temperature was measured at 120 degrees Fahrenheit. There are no cleaning supplies kept in the resident bathrooms.
Common Areas : These included the living room and dining area. The common areas were properly furnished. The dining room table is large enough to seat the four (4) residents, but can sit six.
