Facility Evaluation Report
Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with staff, Romeo Ambrosio, and explained the reason for the visit.
At approximately 9:35am, with the assistance of staff, LPA took a tour of the physical plant. The facility is a one story building. It is licensed to serve level 4I clients, age 18-59. The smoke alarms and carbon monoxide detectors are dual, hardwired and interconnected. There are two fire extinguishers. One is located by the dining room, and the other at the lounge near the front entrance. The charge date is 01/09/25. The last earthquake drill was conducted on 07/31/25. The last fire/disaster drill was conducted on 07/27/25. Complete 1st aid kit and manual is kept in the medication closet.
Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food properly stored. In addition, there is a supply of emergency food and water locked in the hallway closet. Knives are stored in a locked drawer in the kitchen. No cleaning supplies were observed accessible to the clients in care during the day's visit.
Bedrooms: There are four (4) private bedrooms designated for residents' use. All four bedrooms were observed to be properly furnished with appropriate beddings and linens with sufficient lighting. Floors were clean and maintained. Passageways/exits from rooms were clear of obstruction.
Bathrooms: There is a total of three (3) bathrooms designated for residents' use. Bedroom #1 and #4 has it's own private bathroom. The third bathroom is in the hallway. Bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured at 107 degrees Fahrenheit.
