Facility Evaluation Report
In conjunction with a complaint investigation (control #31-AS-20240729183848 ) Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with the administrator, Senami Lateju-Labeodan, and explained the reason for the visit.
At approximately 10:20am, with the assistance of the administrator, LPA took a tour of the physical plant. Required postings were observed in the entry area. The smoke alarms and carbon monoxide detectors are dual. They are hardwired and interconnected. There are two fire extinguishers. One is located by the dining room, and the other at the lounge near the front entrance. The charge date is 02/18/2024. The last earthquake drill was conducted on 05/30/24. The last fire/disaster drill was conducted on 07/27/24
Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility; properly stored. In addition, there is a supply of emergency food and water locked in the hallway closet. Knives are stored in a locked drawer in the kitchen.
Bedrooms: There are four (4) private bedrooms designated for residents' use. All four bedrooms were observed to be properly furnished with appropriate beddings and linens with sufficient lighting.
Bathrooms: There is a total of three (3) bathrooms designated for residents' use. Room #4 has it's own private bathroom. Bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured at 105 degrees Fahrenheit.
Common Areas : These included the living room, lounge and dining area. The common areas were properly furnished. Couch, tables and dining room sets were maintained in good condition. Floors were clean, maintained and no tripping hazard present. The fireplace in the living room is not operational, but is properly screened. No fireplace tools were observed. Exits and passageways were clear from obstruction.
