Facility Evaluation Report

Licensing Program Analyst (LPA), Patrick Shanahan, arrived at the facility in order to conduct an Infection Control Annual. The LPA was greeted by the facility house manager and had his temperature taken and coivid-19 related questions asked before being allowed entry into the home.

The facility has four bedrooms and three clients currently. The fire extinguisher was functional and last serviced in January of 2022. The smoke alarms are not hardwired in the facility, but all were tested and appeared to function properly. The carbon monoxide detectors also functioned properly.

The facility is currently following their infection control plan and no deficiencies were observed during today's visit.
