The inspector’s account

Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with the staff, Rita Balasanyan and explained the reason for the visit. The administrator, Mariam Balasanyan, was notified over the telephone.

At approximately 11:15am, LPA took a tour of the physical plant. The facility is a one story building, licensed to serve clients sixty (60) years of age and above. Required postings were observed in the entry area. The smoke alarms and carbon monoxide detector is dual, hardwired and inter-connected. There is a fully charged fire extinguisher that is located near the dining room.

Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility; properly stored. Knives were stored in a locked drawer. Cleaning supplies were stored locked underneath the kitchen sink.

Bedrooms: There are four (4) bedrooms designated for residents' use. The bedrooms, were observed to be properly furnished with appropriate beddings and linens with sufficient lighting. Exits were clear of hazards and obstruction.

Bathrooms: There are two (2) bathrooms. One bathroom designated for residents, and one designated for staff. The bathrooms used by residents were properly supplied and had functional fixtures. Hot water temperature was measured at 111 degrees Fahrenheit. There were no cleaning supplies or hazardous items observed in the bathrooms.
