Facility Evaluation Report
Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with staff, Genya Sukiasyan, and explained the reason for the visit. The administrator, Nelli Berberyan, was notified over the telephone, but she was unable to attend the annual. .
At approximately 12:00pm, LPA took a tour of the physical plant. Required postings were observed in the living room and entry area. The smoke alarms and carbon monoxide detector are dual, hard wired and inter-connected. The facility has one new fire extinguisher, located by the kitchen. It was purchased on 01/06/26.
Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food properly stored. Knives were stored in a locked drawer. Cleaning supplies were stored locked underneath the kitchen sink.
Bedrooms: There are four (4) bedrooms designated for client use. Bedroom #1 and #4 are shared rooms, Bedrooms #2 and #3 are private rooms. Bedrooms designated for resident use were properly furnished with appropriate beddings, linens, beds, night stand, chairs and dresser. Resident bedrooms have sufficient lighting and closet space.
Bathrooms: There are three (3) bathrooms designated for residents' use. Bedroom #4 has it's own bathroom. All bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured between 107 and 109 degrees Fahrenheit. No cleaning supplies were observed in any of the bathrooms during the day of the inspection.
