Further review also revealed that R1 was accepted back at the facility upon discharged on 01/30/26 from the hospital. LPA's interview with the Administrator revealed that the Administrator was in constant communication with the ERC and hospital staff during the entire hospitalization and worked with R1, hospital, and ERC social worker for the safe return of R1 to the facility.

Based on the information gathered during this and prior visit, the allegation is deemed unsubstantiated at this time.
