Facility Evaluation Report
Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with staff, Oladipo Williams, and explained the reason for the visit.
At approximately 9:00am, LPA took a tour of the physical plant. The facility is a one story building. The smoke and carbon monoxide alarms are battery operated. Both were tested and observed to be operable during the visit. There is a fully charged fire extinguisher located at the medication room, located by the dining room. The fire extinguisher was last charged on May 14, 2025.
Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food properly stored. Knives are stored locked in one of the drawers. Cleaning supplies are kept locked in the garage.
Bedrooms: There are five (5) bedrooms. Four (4) bedrooms designated for residents' use. Two (2) rooms are private, and two (2) are shared. The fifth (5th) room is used as an office. The four (4) bedrooms, in use by residents were properly furnished with appropriate beddings and linens with sufficient lighting.
Bathrooms: There are four (4) bathrooms. Three (3) of the bathrooms were designated for residents' use, and one (1) is designated for staff. Bathroom in use for the residents were properly supplied and had functional fixtures. Hot water temperature was measured between 117 to 119 degrees Fahrenheit.
Common Areas : These included the living room, family room and dining area. Both living room and family room were furnished with sufficient seating of couches, recliners, tables and television. There is a fireplace
