Facility Evaluation Report

Licensing Program Analyst (LPA) Michael Cava attempted to conduct a Required Annual visit at the facility. At arrival, LPA waited at the front door for approximately ten minutes but no answer. LPA called telephone number on file, and was able to get a hold of the administrator, Moses Wakabi. He stated his application for vendorization with Regional Center (RC) is still pending and awaiting approval. Currently there are no residents currently in the home, but he would like to continue to carry his license while waiting for his application with RC gets approved. Facility not operating at this time. Annual fees are current.

LPA observed the home from the outside. Property appears to be vacant, and not operating. There were no cars parked in front of the home.

LPA will make another visit to facility on a later date to complete annual. This report will be left at the front of the home, and will be mailed. Note that LPA conducted a previous annual visit to this facility March 2024, and there were also zero residents at that time.
