Facility Evaluation Report
Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with the LVN, Juliet Luzinda-Ssengo, and explained the reason for the visit.
At approximately 10:15am, LPA took a tour of the physical plant. The facility is level 4 Adult Residential home. The building is one story. Required postings were observed in the entry area. The smoke alarms and carbon monoxide detectors are dual and interconnected. There are two fire extinguishers. One is located in the kitchen, and the other is located in staff office. The charge date for both fire extinguishers is March 14, 2026.
Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food, properly stored. Knives & cleaning supplies are stored locked in the laundry area, which is adjacent to the kitchen.
Bedrooms: There are four (4) bedrooms designated for residents' use. All four bedrooms are private. Bedrooms were observed to be properly furnished with appropriate beddings and linens with sufficient lighting.
Bathrooms: There are three (3) bathrooms designated for residents' use. Bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured between 105 and 107 degrees Fahrenheit. No cleaning supplies were observed in any of the bathrooms during the day of the inspection.
