Facility Evaluation Report
Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with staff/nurse, Tunde Ojelowa and explained the reason for the visit. The administrator, Cassady Stinson, was notified over the telephone and she joined shortly after.
At approximately 9:40am, with the assistance of staff, LPA took a tour of the physical plant. The facility is a one story building that houses level 4I clients. The smoke alarms and carbon monoxide detectors are dual. The smoke alarm was tested and observed to be operable. There are two (2) fire extinguisher in the facility: one (1) in the dining room, and one (1) in the hallway between two bedrooms. Fire extinguishers were observed to be fully charged. Service date was January 19, 2025.
Kitchen: The kitchen was observed to be clean and sanitary. All the toxins, cleaning solutions and disinfectants are locked. LPA did not see any cleaning supplies out and accessible during the day's visit. Knives and sharps were observed to be locked in a kitchen cabinet. LPA found a sufficient amount of perishable and non-perishable food properly stored.
Bedrooms: There are four (4) bedrooms designated for residents' use. Bedrooms are private. All four bedrooms were properly furnished with appropriate beddings and linens with sufficient lighting.
Bathrooms: There are two (2) bathrooms designated for residents' use. Both bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured between 111 to 113 degrees Fahrenheit. No cleaning supplies were stored in the bathrooms.
