Facility Evaluation Report

Licensing Program Analyst (LPA), Patrick Shanahan, arrived at the facility in order to conduct an annual inspection. LPA was greeted by facility staff, who took the LPA's temperature and asked COVID-19 symptom questions before being allowed entry. Staff then called the administrator and he arrived a short while later.

LPA was able to test the carbon monoxide detector and smoke alarms. All appeared to be functioning properly. The fire extinguisher was serviced on 4/1/2021. All door alarms were tested and functioned properly.

No health and safety concerns were observed during today's visit.
