Facility Evaluation Report
Licensing Program Analyst (LPA) Michael Cava conducted a Case Management (CM) visit to the facility to follow up on deficiencies pertaining to complaint control #31-AS-20250423110515. LPA met with staff, Arnold Makura, and advised him of the visit. The administrator, Lala Soghomonyan was advised over the telephone.
On 05/01/25, a complaint visit was made, and the facility was issued citations for sections 87202(a)(1)- Fire Clearance and 87608 (5)(B)- Postural Support, with a plan of correction date due 05/08/25. Today's visit was to insure corrections have been made.
Regarding 87202 (a)(1)- Fire Clearance, licensee is in the process of submitting the request to obtain the fire-clearance to retain non-ambulatory clients. POC still due by end of close of business (COB)
Regarding 87608 (5)(B)- Postural Support, licensee replaced the bedrails for Resident 4 (R4) to comply with 87608(5). "A bed rail that extends from the head half the length of the bed and used only for assistance with mobility shall be allowed". Citation will be cleared.
In addition to following up with the citations, LPA made a physical plant inspection to insure the health and safety of the residents in care. No immediate health and safety concerns noticed at this time. LPA did observe two hospice nurse at the facility at the time of the visit.
