residents and would not be reimbursed. The refrigerator was checked and there was only food for 1 resident brought by the family member. On 02/18/2026, upon facility inspections LPAs noted that facility does not have sufficient quantity and variety of perishable and nonperishable food supply. Refrigerator was almost empty. Staff verified that facility has low food supply. S1 stated that they were buying the food out of her pocket and was not getting reimbursed. All facility residents were interviewed during Case management visit that was conducted in conjunction with complaint investigation and they address their concerns about food service and insufficient food supply. At the time of this visit at 8:40a.m. LPA Alvizar – Ettima also spoke with the staff and residents and they revealed the same information as during initial visit.
Based on inspection, observation and interviews it was concluded that the facility has insufficient food supply. Therefore, the allegation is SUBSTANTIATED at this time.
Lack of supervision resulted in resident AWOL
It was alleged that a several residents wandered away from the facility and a missing person report was filed. Resident was found down the street by law enforcement.
At the time of this visit at approximately 10:00a.m. LPA Alvizar- Ettima spoke with staff and they stated that residents had mental issues and walked out the door. Upon inspection, it was observed that there was only one (1) staff during the times residents AWOL. Residents resided at the facility for several days. S1 indicated that they ran out looking for residents in the street and did not find them. S1 did not have the resident’s facility file. At 10:40a.m. residents present at the facility were interviewed and they had no knowledge about any residents wandering away from the facility. Facility did not have resident’s facility records at the time of this visit.
Based on inspection, observation and interviews there is a sufficient information to verify the allegation. Therefore, the allegation is Substantiated at this time.
