Facility Evaluation Report
This report is being created to address an incident pertaining to complaint control # 31-AS-20260119231501. . It was reported that Resident 1 (R1) had a fall the night of 01/16/26, but facility did not provide immediate medical attention after the fall. As a result, R1 sustained a broken hip and dislocated shoulder. Interview with Staff 1 (S1) confirmed paramedics were not called immediately to treat R1 for their fall. When S1 was questioned why 911 emergency services was not called, S1 stated they were not called for fear that licensee/operator will be upset and threaten them, like before. S1 did what they could’ve done best, by staying alongside R1 until family can call 911.
Although S1 remained by R1 to provide some support after R1’s fall, immediate medical attention was not provided to R1, as R1 sustained a serious injury as a result of this fall. A citation will be issued on the 809D for failure to provide adequate care.
