Facility Evaluation Report

At approximately 9:00 a.m. on 03/03/26, Licensing Program Manager (LPM) Naira Margaryan and Licensing Program Analysts (LPAs) Michael Cava, Antonia Alvizar-Ettima, and Nicholas Reed conducted an unannounced case management visit in conjunction with a subsequent complaint visit. LPAs and LPM met with Staff #1 (S1) and disclosed the reason for the visit.

Today’s case management visit was conducted to address previously noted concerns noted in the facility. On 10/20/25 the Department issued an order of immediate exclusion for the facility administrator Lala Soghomonyan. LPAs Michael Cava and Jose Gary Tan hand-delivered the immediate exclusion letter to staff Matthew Caceres. Matthew informed LPAs that the Licensee was in the process of hiring a new certified administrator to oversee facility operations. During that visit, LPAs noted various Title 22 deficiencies and issued citations and civil penalties.

On 02/18/26 LPAs Michael Cava and Leslie Ngo-Castaneda conducted another Case Management visit to the facility to ensure a new Administrator was hired. Between 8:30 a.m. and 10:00 a.m. on 02/18/26 LPAs interviewed S1 and four (04) residents. Nobody knew who the licensee or administrator were. Between 9:30 a.m. and 10:30 a.m. on 02/18/26 LPA Cava made multiple attempts to contact the licensee on file but received no response.

On 02/26/26 LPA Cava made a Case Management visit and met with the same facility staff, S1. All residents were observed still residing in the facility. Additional civil penalties were issued. S1 was again reminded that all residents must be relocated. No deficiencies were corrected as of 02/26/26 and today. Additional civil penalties were issued for repeat violations and fire clearance violations. LPA Cava issued a “7-day notice of closure” to the Licensee via regular and certified mail and email, informing that the Licensee must contact the Department within seven (07) days. Failure to respond would consider the license forfeited.
