Facility Evaluation Report

Licensing Program Analysts (LPAs) Leslie Ngo and Michael Cava conducted a Case Management visit to facility to follow up and see if the licensee has hired a qualified administrator, Plan of Corrections (POCs) issued on 02/18/26 and if the licensee is in receipt of the seven day letter mailed on 02/24/26. LPAs also conducted a physical plant inspection and health and safety check to insure facility compliance to title 22 (made between 10:11am - 10:50am), and interviews with residents and record review (10:50am - 11:45am).

Based on observation, Licensee has failed to correct POCs. Moreover, additional deficiencies were observed. Citations issued on the 809D
